Summary of Information for Colorado Referring Professionals

 

  1. Who can apply to Friends of Man?  
    A referring professional (social worker, case manager, clergy, physical therapist, patient navigator, school personnel including teachers, counselors, nurses)  can apply to Friends of Man on behalf of his/her client or patient. The applicant can help to provide the necessary information once the referring professional has received the application.
  1. Who is eligible for Friends of Man assistance?   
    We assist applicants who have:
      • Household income of 300% or less of Federal Poverty Guidelines
      • Been in a stable living environment for at least 3 months
      • Are not homeless or living in institutional care
      • U.S. Government-issued ID:
        • Social Security number                   
        • ITIN
        • Protected status, or other U.S. government-issued document
        • If none of the above, we can consider documented proof of stable residency/employment for one year
  1. What does Friends of Man help with? 
    We have two application forms for requests either over or under $500:

         1. General Program Application – Requests over $500

·         Hearing aids

·         Home modifications (no walk-in tubs)

·         Wheelchair ramps

·         Mobility equipment (wheelchairs, scooters, lifts, etc.)

·         Daycare for currently employed applicants

·         Cataract surgery

·         Assistive technology (medically prescribed)

·         Prostheses

·         Medical equipment (Bioness, lift chairs)

·         Therapy camp for children

·         Vehicle adaptations, hand controls (no van conversions)

·         Dentures and extractions

·         Partial dentures and fillings for employed applicants

·         Rent assistance for employed applicants (call us first)

·         Medical needs

         2. Basic Needs Program or Other Items Under $500

      • Basic Needs Program: Food, clothing, household essentials, personal needs
      • Items under $500: Glasses, orthopedic braces and shoes, compression garments, prescriptions, other medical needs, food and gas during medical treatment                            
  1. Is there anything that Friends of Man does NOT help with?
      • Back bills (already paid, ordered, received, or work started)
      • Utilities
      • Exams or diagnostic testing
      • Items not listed above in our two Program descriptions
  1. What is the Friends of Man Payment Policy?
    When a request is approved, Friends of Man notifies the vendor in writing of our commitment. The vendor provides the item or service to the applicant only after receiving our written approval. Friends of Man then pays the vendor directly, after delivery of the item or service is confirmed.
  2. How long does the application process take?  
    Depending on how quickly we receive all the requested information and documentation, a decision could be reached in as little as two to three weeks.
